ERM vs VMT is a common OCT question because both conditions distort the macula.
An epiretinal membrane (ERM) pulls tangentially across the inner retinal surface.
Vitreomacular traction (VMT), however, pulls anteriorly through an adherent posterior hyaloid.
Therefore, the direction of force explains most differences in imaging and management.
ERM vs VMT: The Basic Mechanism
An ERM forms when glial cells proliferate on the internal limiting membrane.
The sheet then contracts and wrinkles the inner retina.
VMT follows an incomplete posterior vitreous detachment.
The hyaloid stays attached at the fovea, so it distorts the foveal center.
Who Develops Each Condition
Both become more common with age.
Secondary ERM may follow retinal tears, uveitis, vascular occlusion or intraocular surgery.
VMT can also coexist with diabetic macular edema or high myopia.

Both conditions cause metamorphopsia, blurred central vision and reading difficulty.
However, many mild ERMs cause no symptoms.
VMT may cause a central scotoma when cysts or subretinal fluid develop.
Consequently, symptoms alone cannot separate the two.
Slit Lamp and Fundus Clues
ERM often shows a cellophane sheen, retinal striae and tortuous vessels.
VMT, by comparison, can look subtle, with only a dull foveal reflex.
Therefore, a near-normal fundus does not exclude traction.
OCT Appearance of ERM
OCT shows a hyperreflective band on the inner retinal surface, often with folds.
The foveal pit may flatten as the retina thickens.
Govetto and colleagues proposed a four-stage scheme based on ectopic inner foveal layers.
Advanced stages carry a less favorable visual outlook.
(For a broader overview, see how serious an ERM can be.)
OCT Appearance of VMT
OCT shows a thin hyaloid band that inserts at the fovea.
The International Vitreomacular Traction Study classification requires perifoveal separation plus foveal structural change.
Attachment without retinal change is vitreomacular adhesion, a different finding.
Pseudocysts, foveal elevation and subretinal fluid may then follow.
ERM vs VMT: Reading the Scan
First, trace the hyaloid across the whole macular volume, because one line can miss it.
Next, inspect the inner surface for a hyperreflective membrane.
A tented fovea favors VMT, while a broad thickened macula with wrinkling favors ERM.
Finally, check the ellipsoid zone to frame prognosis.
When Both Appear Together
ERM and VMT often coexist on the same scan.
A coexisting ERM may also predict a poorer response to ocriplasmin.
Therefore, report both findings.

Conditions to Exclude
A pseudohole may resemble a full-thickness macular hole, yet OCT shows intact outer retina.
Similarly, cystoid spaces can reflect diabetic or postoperative edema without traction.
In secondary ERM, a dilated peripheral examination also looks for tears.
Natural History in ERM vs VMT
Most idiopathic membranes progress slowly and often remain stable.
VMT behaves less predictably because the attachment can release or tighten.
Spontaneous release occurs in some eyes, and acuity may then recover.
Meanwhile, persistent traction may worsen cysts or progress to a macular hole.
Hikichi and colleagues documented visual decline in many unreleased eyes.
ERM vs VMT: Observation and Surgery
Observation suits mild ERM with good acuity and little distortion.
It also suits many early VMT eyes with mild symptoms.
However, VMT needs closer review because a macular hole can develop.
ERM surgery usually follows distortion or functional decline, with no fixed acuity threshold.
For VMT, surgeons consider vitrectomy when symptoms and structural change worsen.
What Each Operation Does
ERM surgery combines vitrectomy with membrane peeling, and many surgeons also peel the internal limiting membrane.
VMT surgery separates the hyaloid from the fovea.
Both operations can reduce distortion, but neither guarantees normal vision.
Risks include cataract, retinal tears, detachment and rare endophthalmitis.
Macular pucker recurrence can also occur after peeling.
Nonsurgical Options for VMT
Ocriplasmin released VMT in 26.5% of treated eyes, compared with 10.1% after placebo, at day 28 in MIVI-TRUST.
However, availability varies, and reports describe transient visual changes.
Pneumatic vitreolysis is another option, although tears and detachment remain concerns.
Neither approach removes an established ERM.
ERM vs VMT Counseling
Tell patients with ERM that surgery aims to reduce distortion, and recovery may take months.
Patients with VMT should know that release is possible, but a macular hole can also develop.
In addition, advise prompt review for sudden scotoma, flashes or a curtain.
Pitfalls to Avoid
- Never diagnose VMT from one foveal OCT line
- Adhesion without retinal change is not true traction
- A coexisting ERM is easy to miss on a traction scan
- Pseudoholes need OCT before anyone calls them macular holes
- Peripheral retina examination is needed in secondary ERM
