Bitot spots are keratinized, foamy patches on the bulbar conjunctiva that mark one of the earliest clinically visible signs of vitamin A deficiency. They sit classically in the interpalpebral zone, usually temporal to the limbus, and represent a buildup of desquamated keratinized epithelium and gas-forming bacteria (often Corynebacterium xerosis) on a conjunctiva that has lost its normal mucin-secreting goblet cells.

The sign matters because it is cheap and fast to recognize in the field, and because it flags a child or adult who is at real risk of corneal involvement and permanent blindness if the underlying deficiency is not corrected.

What Is Vitamin A Deficiency Xerophthalmia?

Vitamin A (retinol) is required for the normal differentiation of conjunctival and corneal epithelium, and for regeneration of rhodopsin in the rod photoreceptors. Xerophthalmia is the umbrella term the World Health Organization uses for the full spectrum of ocular findings caused by vitamin A deficiency, from night blindness through conjunctival and corneal xerosis to keratomalacia.

Bitot spots sit in the middle of that spectrum. Their presence signals conjunctival xerosis (WHO classification X1B) rather than the earliest stage (night blindness, XN) or the sight-threatening later stages that involve the cornea.

Who Is Affected

Xerophthalmia remains a public health problem in parts of South and Southeast Asia and sub-Saharan Africa, concentrated in young children with inadequate dietary intake of preformed vitamin A or provitamin A carotenoids, often compounded by measles, chronic diarrhea, or protein-energy malnutrition. In high-income countries, the deficiency is seen almost exclusively in the context of fat malabsorption — cystic fibrosis, short bowel syndrome, chronic pancreatitis, cholestatic liver disease — or after bariatric surgery when supplementation is neglected.

Bitot spots themselves can persist after the systemic deficiency has been corrected, since the keratinized epithelium takes time to resolve even once retinol stores are replete. This makes the spot a marker of past or present deficiency rather than proof of an active, untreated state.

Clinical Presentation

Patients, or more often their caregivers, report dryness and a gritty foreign-body sensation rather than pain. Night blindness is frequently present first and is easy to miss unless specifically asked about, since young children rarely volunteer it.

On examination the conjunctiva appears dull, thickened, and non-wettable rather than its normal smooth, glistening surface. The Bitot spot itself is a raised, silvery-white or foamy patch, triangular or oval, with a crumbly surface that can sometimes be partly wiped away, unlike a pinguecula or pterygium.

Exam Findings and Differential Diagnosis

  • Pinguecula — yellowish, vascularized, usually nasal, not foamy
  • Pterygium — fibrovascular growth encroaching onto the cornea
  • Conjunctival concretions — small, hard, yellow-white deposits, usually palpebral conjunctiva
  • Squamous metaplasia from chronic ocular surface disease (dry eye, cicatricial conjunctivitis)
  • Superficial limbal keratitis

The distinguishing feature of a true Bitot spot is the clinical context: a patient, usually a child, with signs of malnutrition, night blindness, or an underlying malabsorptive condition. Conjunctival impression cytology, when available, shows loss of goblet cells and keratinizing squamous metaplasia, which supports the diagnosis when the picture is not classic.

Diagnostic Evaluation

The diagnosis is largely clinical, based on the appearance of the conjunctiva together with a history that fits vitamin A deficiency. Serum retinol below 0.70 µmol/L (20 µg/dL) confirms deficiency, though levels can be falsely normal during acute infection because retinol-binding protein is an acute-phase reactant.

A relative dose-response test, which measures the rise in serum retinol after an oral vitamin A load, is more sensitive for detecting marginal stores but is rarely needed outside research settings. In a child from an endemic area, response to treatment is often used as a practical confirmatory test.

Management

Treatment follows the WHO protocol: high-dose oral vitamin A (200,000 IU for children over one year, 100,000 IU for infants) on the day of diagnosis, again the following day, and a third dose two to four weeks later. Underlying malabsorption or dietary insufficiency needs to be addressed in parallel, or the deficiency will simply recur.

Lubrication with preservative-free artificial tears helps the ocular surface while epithelial healing catches up, but it does not substitute for systemic repletion. The Bitot spot itself often takes weeks to months to flatten and can leave a subtle residual dulling of the conjunctiva even after the deficiency has resolved, so its persistence alone should not be read as treatment failure.

Complications and Prognosis

Left untreated, conjunctival xerosis can progress to corneal xerosis, corneal ulceration, and keratomalacia — a rapidly progressive, often bilateral corneal melt that is a leading preventable cause of childhood blindness worldwide. Measles infection accelerates this progression sharply, which is why the WHO recommends vitamin A supplementation for all children hospitalized with measles in deficient regions regardless of documented deficiency.

Once corneal involvement occurs, vision loss can be permanent even with prompt treatment, which is the main argument for recognizing the conjunctival stage early. The mucosal changes underlying xerophthalmia also raise susceptibility to respiratory and diarrheal infections, so the ocular sign is a marker of broader systemic vulnerability, not an isolated eye problem.
